Fixed numerous bugs:

- Sleep and other timed duration spells now wear off on mobs (oops)
- Demoness portal should alway drop now, it was a problem with limited items flagged del@maint not poofing at cleanup properly
- Boss regen is fixed for those bosses that weren't supposed to be regenning so often.  This was an interesting one.  AED and a couple other bosses share a "normal" regen slot with other non-bosses which is kind of strange since almost every other boss has their own slot.
- Fixed a bug which allowed 2 spell rounds to fire in one round after some normal combat rounds.
- Made a change to the combat round offset system.  I thought I had it only changing per map previously, but discovered in the code it was offsetting every room which caused lots of places where problems could occur (ie. not being able to heal for several rounds while running).  We'll give this a shot, if it's too problematic we'll do a full roll back to just one single combat round for the entire realm.  I know this will upset a few of you, but please just give it a try.
- Bliz and other spells with endcast spells weren't doing another roll before applying the endcast spell's abilities.  So bliz's intitial damage would roll for like 270, then the endcast freezing effect would also do another 270 damage rather than the range.  The duration is also recalculated, so the freezing effect now actually is a DOT spell.
- I verified shop regen appears to be working.  Please let me know if you notice anything wrong with it.

Found and fixed a bug with DOT spells causing death messages to show over and over.
Area spells should now break if no mobs are in the room.

Took a stab at fixing the longstanding name/password login issue we've had where Mega repeatedly sends the name and password in reverse order.
