I noticed I had a markus head on me but not in inventory
I could not drop it.

I could though remove it.  then it disappeared.

I finished the 1st evil quest with it still being worn by mistake/not thinking.   It worked though. got ring and 150k.  I just noticed this head on me.  Could the presence of this head have fouled up my character from getting 2nd evil done properly?

We'll get it straightened out this weekend for sure.

I'm not in front of the code right now, but my gut feeling is this is probably not related to the second quest issue.  The second quest issue appears to be a flaw in the logic in the textblock code.  I'll likely fix both this weekend.
