If you go with making SMASH a utility, does it fire in place of an attack round or between rounds? 

I would go for between rounds and run it through the spellcasting processing in that it can be resisted.  It would also casting level equal to inverse of the level that you receive it in that it is a level 27 "spell" for a witch hunter and a level 20 "spell" for a cleric or warlock and spell immunity would apply.

Question 1: Should smash be primarily a damage ability, or primarily a utility ability?
-Both; at least one will fire, with an adjustable % ratio, but also a small % that both can fire. Higher max damage should have negative affects on ACC, maybe for every 10 you get -1 acc for the damage roll

Question 2: Should witchunters be forced to passively resist friendly spells?
Yes. Mres should simply apply to healing the same way it does to damage.  Witchies can achieve "OP" status already with optimal gear/stats. Also the bonus is exploitable (through friendly party members).

Question 3: Should trainers cost money
No, not until the economy is dealt with. Right now money costs are just a nuisance.

Question 4: Should the changes following this question be made to the leather armour pool?
Yes; Those look like good changes and leather needs work. Also I recommend 'mass introduction' of new item content since people will whine unbalancing out of existance; it's a self contained injection. That and everyone here knows more or less what right looks like so newly introduced items won't even be that far out of balance.
